ORGANIZER
The Cronos Med Loyalty Program is organized by Clinica Medical Service S.A, based in Bacău, Str. 9 MAI, Nr. 32, Block 32, Stairway A, Floor P, Ap. 4, County Bacău, registered with the Trade Register Office under no. J04/1901/1991, unique registration code RO 985416, hereinafter referred to as the Organizer. The Loyalty Program includes eligible services provided by the Organizer and affiliated companies listed in Annex 1, hereinafter referred to as Partners. The list of Partners may be unilaterally amended by the Organizer.
Services and products described in Annex 2.
Participants in the Loyalty Program are bound by the terms and conditions of the regulation. The rules are available on the website www.conosmed.ro. The organizer reserves the right to modify or change these rules, with the changes coming into force after their publication on the website www.cronosmed.ro.

THE RUNNING AND DURATION OF THE PROGRAM
The Loyalty Program is organized and takes place on the territory of Romania in the working points of the Organizer and Partners. The Loyalty Program has an indefinite duration, with the exception of certain periodic campaigns. The Organizer has the right to decide to terminate or modify the Loyalty Program at any time, without giving prior notice. Notice of termination of the Loyalty Program will be posted by the Organizer on the website www.cronosmed.ro.

CONDITIONS FOR PARTICIPATION
Any natural person who is 18 years of age at the date of applying for participation in the Loyalty Program, following at least one purchase transaction of goods / services at any point of work of the Organizer / Partner, can be a member of the Loyalty Program.
You can participate in the loyalty program by filling in and signing the registration form at any of the Organizer's and Partner's points of work. The signed form will be handed over to the Organizer/Partner's employee, who will verify the data and activate the card, in case of correct completion of the form.
The participant is obliged to provide all the data required to fill in the mandatory fields of the form: name, surname, date of birth, gender, mobile phone, e-mail address and signature.
Unsigned or incomplete or erroneous registration form is not valid, which entails the impossibility to participate in the loyalty program and the non-issuance of the Cronos Med loyalty card or its withdrawal and cancellation of the participant's status.

MEMBERSHIP CARD
The membership card is the property of the Organiser.
The card can be issued in physical or digital format, at the Organizer's option, and can be used to obtain the Benefits set out in Article 5 after a period of 24 hours from its issuance.
The membership card is not a payment instrument and does not have a payment account associated with it, its purpose being to record the participants and the benefits granted under the Loyalty Program, based on the digital code associated with each card.
The membership card is not nominal. However, the membership card is not transferable.
The Organizer/Partner has the right to invalidate any loyalty card after 12 months from the last use or after use in violation of the provisions of this Regulation or legal provisions.
If a loyalty card has been damaged, stolen or lost, the event must be reported to Cronos Med on 021 9097 in order to invalidate the card. The holder is entitled to a new loyalty card.
The Loyalty Program Participant is responsible for the integrity of the card and compliance with the Loyalty Program participation conditions.

BENEFITS GRANTED
The valid Cronos Med Loyalty Card allows participants to receive the following benefits during the Loyalty Program:
- Direct discount of up to 10% (not more than 500 lei) from the price of services provided by the Organizer or Partners, on the first purchase after enrolling in the Loyalty Program. The benefit does not apply to surgical medical services.
- Direct discount of up to 5% on physical products sold by the Organizer and/or Partners.
- Direct discount of up to 20% (no more than 1,000 lei) on the first purchase of non-invasive services during the week of the owner's birthday.
The discount is calculated by applying the percentage to the base price of the services or products purchased in a single transaction. The base price is the price charged by the Organizer/Partners including VAT, without any other discounts.
The direct discount through price reduction applies to services and products provided that the final sales price (resulting from the discount granted) is higher than the purchase cost of the product, in accordance with the legal regulations in force. If, when the discount is applied, the selling price of the services and products falls below or equals the purchase cost, the discount will be reduced accordingly so that the final selling price of the products and/or services is higher than the purchase cost.
The direct discount will not be cumulated with any other ongoing promotion or discount on the same services or products, unless expressly decided by the Organizer.

CARD USE
In order to use the card and receive the benefits, the participant must present the Organizer/Partner's employee with the card and ID card. The membership card will be scanned in order to grant the benefits available on the occasion of cashing the price of services/products. The Participant undertakes to keep the card safe and not to transfer it to third parties or allow its use by third parties.
The Organizer/Partner has the right to verify, on the basis of the electronic code associated with the card, the transactions carried out using the card. The Organizer/Partner has the right to retain and cancel any card used fraudulently. The Organizer/Partner shall not be liable for the impossibility of granting benefits as a result of the Participant's failure to comply with the Loyalty Program conditions.

PROTECTION OF PERSONAL DATA
By signing the registration form, participants agree to the provisions of these rules. By signing the registration form, the participant expressly consents to the inclusion of personal data in the database of Cronos Med clinics and partners, as well as to the processing of such data for the purpose of granting benefits according to Cronos Med policy, for statistical purposes, and, if he/she has given his/her consent, for direct marketing, profiling and to receive personalized information materials from Cronos Med and partners in the loyalty program. By joining the loyalty program, the personal data of the participants will be processed for commercial communications related to the loyalty program, based on the organizer's commercial policies.
The Organizer undertakes to comply with the provisions of the European Regulation 679/2016 on the protection of personal data for the duration of the Loyalty Program and thereafter. Thus, the Organizer undertakes to keep the personal data of the Loyalty Program participants confidential and to use them in accordance with this Regulation and the legislation in force.
The organizer processes the personal data of the loyalty program participants in accordance with the provisions of European Regulation 679/2016 (GDPR) on the protection of natural persons with regard to the processing of personal data and on the free movement of such data.
The provision of personal data by the participant is optional, however, refusal to provide all personal data requested for the purpose of enrolling in the loyalty program leads to the inability to access the benefits of the loyalty program.
The organizer will process the name, surname, card number and purchase history in order to analyze transactions that may be fraudulent in the legitimate interest of the organizer.
The organizer will process the name and surname, card serial number, purchase history, location where the card was used, telephone number, e-mail address, based on its legitimate interest to improve the quality of the services offered.
The personal data of participants enrolled in the loyalty program will be processed for 12 months from the date of last use of the Loyalty Card or until the date of withdrawal of consent, whichever comes first. The personal data of participants enrolled in the Loyalty Program will be stored for 2 years from the date of invalidation of the Loyalty Card or the date of withdrawal of consent. If, after the expiry of that period, the organizer determines that it has a legitimate interest or legal obligation to further process the personal data of participants for other purposes, the organizer shall duly inform the participants accordingly.
In the context of the processing of personal data of participants enrolled in the loyalty program, they have the following rights:
- Right of access to the personal data processed: participants have the right to obtain confirmation as to whether or not personal data are processed and, if so, to have access to the type of personal data and the conditions under which they are processed, by sending a request to the organizer.
- Right to request rectification or erasure of personal data: participants have the possibility to request, by sending a request to the organizer, the rectification of inaccurate personal data, the completion of incomplete data or the erasure of personal data where
(i) the data are no longer necessary for the original purpose (and there is no new lawful purpose),
(ii) the legal basis for the processing is the consent of the participant and the participant withdraws his/her consent and there is no other legal basis,
(iii) the participant exercises his/her right to object and the organizer has no legitimate grounds for further processing,
(iv) the data have been processed unlawfully,
(v) erasure is necessary to comply with European or national law; or
(vi) data have been collected in connection with information society services offered to children (if applicable), where specific consent requirements apply.
- Right to request restriction of processing: participants have the right to obtain restriction of processing in cases where:
(i) considers that the personal data processed are inaccurate, for a period enabling the organizer to verify the accuracy of the personal data;
(ii) the processing is unlawful, but the participant does not want his/her personal data to be erased, but the use of such data to be restricted;
(iii) where the organizer no longer needs the personal data for the purposes referred to above, but the participant needs the data in order to establish, exercise or defend a legal claim, or
(iv) the participant has objected to the processing, for the period of time for the organizer to verify whether its legitimate grounds override the rights of the participant.
- The right to withdraw consent to the processing, where the processing is based on consent, but without affecting the lawfulness of the processing activities carried out up to that time. By withdrawing consent to participate in the loyalty program, the participant will no longer be able to access the benefits granted under the Cronos Med policy and, as a result of the withdrawal of consent, the loyalty card will be invalidated.
- The right to object to the processing of data on grounds relating to the particular situation of the Participant, where the processing is based on legitimate interest, and to object at any time to the processing of data for direct marketing purposes, including profiling.
- The right not to be subject to a decision based solely on automated processing, including profiling, which produces legal effects concerning the Participant or similarly affects him/her in a significant way.
- The right to data portability, meaning the right to receive the personal data you have provided to the organizer in a structured, commonly used and machine-readable form and the right to have the data transferred to another controller where the processing is based on consent or the performance of a contract and carried out by automated means.
- The right to lodge a complaint with the Data Protection Authority (ANSPDCP) and the right to apply to the competent courts.
The above rights can be exercised at any time. To exercise these rights, participants may send a request via e-mail to office@cronosmed.com. In the request, the participant may indicate whether he or she wishes the information to be communicated to him or her at a specific address, which may be an electronic mail address or a mail service which ensures that it is delivered only personally.
In addition, if the participant wishes to withdraw their consent for direct marketing purposes, they can use the "unsubscribe" option that is included in every newsletter marketing communication.
All personal data processing activities set out in this Regulation relate exclusively to persons who are at least 18 years of age.

OFFICIAL RULES
The rules are available for free consultation to any Loyalty Program Participant in all Cronos Med clinics and partner locations, as well as on the website www.cronosmed.ro. By participating in this loyalty program, participants agree to abide by and comply with the terms and conditions of this Regulation.

- Medical surgical services
In exceptional circumstances, the Organizer or the Partner may unilaterally decide that certain Products / Services cannot be included in the Loyalty Program.
